In the Commonwealth of Pennsylvania, the age to consent for services and parental involvement is 14. If your teen is 14 or older, they have the right to decide the level of parental involvement for treatment. As a result, it is best if you involve your teen in obtaining information and their signature on the forms below, in addition to your signature.
In the Commonwealth of Pennsylvania, parents with shared custody must both agree to their child (who is under the age of 14) receiving therapy. Both parents MUST bring a completed Consent for Treatment form along with a copy of their custody agreement to The Center prior to meeting with a therapist.
